Free root cause analysis from Aruvi Consultancy Services. Guided workflow: Problem → Pareto → Fishbone (6M) → 5 Whys → Report.
From Free Tools, open RCA Tool. Create a new analysis or click Load demo sample (Wire Settlement Delay) to explore a complete example.
Figure 1 — Home: create a new RCA or load the demo sample.
On Problem, name the RCA, set status/owner/tags, and write a problem statement that includes impact and context. The sidebar shows completion % as you progress.
Figure 2 — Problem definition: clear statement stakeholders can agree on.
Add causes with frequency and impact (1–10). Bars highlight the vital few so the room focuses effort.
Figure 3 — Pareto: frequency × impact ranking.
Use Man, Machine, Material, Method, Measurement, and Environment categories. Aim for several solid causes — the progress tracker expects at least three.
Figure 4 — Fishbone (Ishikawa) by 6M category.
Answer each Why, then write a clear root-cause statement that is controllable and specific.
Figure 5 — 5 Whys path and root cause statement.
On Report, add prioritized countermeasures and an action plan. Download Markdown, use Print / Save as PDF, or Export JSON from the top bar to keep a portable file.
Figure 6 — Report: actions, countermeasures, and exportable summary.
RCA content stays in this browser until you export. Nothing is uploaded to Aruvi servers for storage. Treat exported JSON and Markdown as confidential investigation material. See Free Tools for the full note.
